Capital Health and Wellness provides mental and behavioral health services, including assessments, counseling, medication management, Intensive Outpatient Programs, and related support. All services are subject to clinician availability and applicable medical guidelines.

3. Appointment Booking & Payments

• Booking: Appointments can be scheduled online or by phone. Confirmation occurs once insurance eligibility is verified (if applicable) and intake forms are completed.
• Fees & Billing: Current service fees are posted on our website. Payment is due at the time of service unless prior arrangements are made. We accept most major insurance plans and will assist in claims submission. You remain responsible for any co‑payments, deductibles, or non‑covered services.
• Cancellations & No‑Shows: To avoid a cancellation fee, please provide at least 24 hours’ notice. Late cancellations or no‑shows may incur charges up to the full session fee.

4. User Responsibilities

• Provide accurate and complete information during intake and at each visit.
• Respect our clinicians and staff; abusive behavior may result in termination of services.
• Follow prescribed treatment plans and report any changes in health or medication.

5. Intellectual Property

All content on our website—text, graphics, logos, images—is the property of Capital Health and Wellness or its licensors and protected by copyright and trademark laws. You may not reproduce, distribute, or create derivative works without our express written permission.
